Privacy Policy

This Privacy Policy describes how our office manages to safeguard and utilise your safeguarded health information (PHI). It is the law that we protect your medical information and tell you what we do concerning privacy. PHI contains data that identifies you and pertains to your past, present, or future physical or mental health, and any health care services that are associated with it.

This policy outlines the manner in which we can use and disclose your PHI to treat and make payments in health care activities. Any other use or disclosure will only be done with your written consent except those allowed or mandated by law. This policy further elaborates how you have rights to your health information, access and control.

How We Use and Disclose Information

We can use your PHI without permission to the following purposes:

Treatment: To offer, plan, or administer your health care, such as referrals to other providers.

Payment: To send bills and receive money of vague ones or insurance companies in relation to rendered services.

Health Care Operations: To facilitate the running of the office like quality improvement, training of staffs, business planning and appointment reminders.

Your name can be called in the waiting area,- sign-in sheets can be used or can call you regarding appointments. In the case of service providers who are third parties, we need a written agreement that would ensure your privacy.

The written authorization is required in any other usage of your PHI. Such authorization you may withdraw at any time in writing.

Your Rights

You have the right to:

See and get copy of your medical records.

Ask about limitations on use or sharing of your information.

Ask them to use confidential means of communication.

Ask to have your records corrected.

Be presented with a list of some disclosures.

The requests are supposed to be presented to the Office Manager. There are requests that can be rejected within the law.

Complaints

In the case that you feel that your rights to privacy have been infringed, you can complain to our office or to the U.S Department of Health and Human Services. Filing a complaint will not be revengeable.
